Terms & Conditions
1. About FORMA
FORMA is a private Pilates practice operated by two independent instructors.
While we work under a shared brand, each instructor operates as an individual sole proprietor and is solely responsible for the sessions they deliver.
When you book a session, you enter into a direct client relationship with the instructor leading that session.
You are welcome to book with either instructor, and we may occasionally coordinate schedules or cover sessions for one another when needed.
2. Our Sessions
We offer:
Introductory sessions
Single sessions
4-session packages (over 4 weeks)
8-session packages (over 4 weeks)
All sessions are private and tailored to you.
We work across different Pilates equipment, including reformer, chair, tower, mat, and small props.
Introductory session requirement
All new clients are required to complete an introductory session before participating in ongoing training or packages.
This session allows us to assess movement patterns, posture, and individual needs in order to provide safe and appropriate guidance.
We do not offer fully tailored programming without this initial assessment.
We reserve the right to postpone or limit training if the introductory session has not been completed.
3. Booking & Payment
Sessions are booked in advance and confirmed once payment is received.
Payment is made via bank transfer based on invoice
Packages are paid in full before the first session
A regular weekly time is recommended to keep consistency, but this can remain flexible where needed
4. Package Structure
Packages are designed to support regular, consistent practice.
Packages are valid for 4 weeks from your first session
When possible, we keep a consistent weekly slot for you
Adjustments can be made depending on availability
If you have upcoming travel or time away, just let us know in advance — we’ll make sure your sessions can continue smoothly around it.
5. Cancellations & Scheduling
We keep the structure simple and respectful of both your time and ours.
Sessions can be cancelled or rescheduled with at least 24 hours’ notice
Late cancellations are counted as used sessions
Missed sessions (no-shows) are also counted as used
When you cancel on time, we’ll always do our best to find another slot for you within the same week where possible.
If we ever need to cancel, your session will be rescheduled and your package extended so nothing is lost.
6. Absence & Holidays
If you’re planning time away, simply let us know in advance — we’ll extend your package so everything stays aligned with your schedule.
If we are away or the studio is closed, your sessions will naturally continue afterward, with no loss of value.
7. Your Health & Responsibility
Before your first session, you’ll complete a short health intake form.
We rely on you to share anything relevant that may affect your training, including injuries, medical conditions, or recent changes in your health.
If something doesn’t feel right during a session, you are always encouraged to stop and communicate.
We may also pause or adjust a session if something appears unsafe — your wellbeing always comes first.
Failure to disclose relevant medical information may affect our ability to ensure your safety and may limit our liability to the extent permitted by law.
8. Medical Considerations
If you have a medical condition, are recovering from injury or surgery, or have recently undergone physiotherapy, you must ensure that you are cleared to resume physical activity by a qualified healthcare professional before attending sessions.
Where you have been referred by, or are under the care of, a physiotherapist or other healthcare provider, you are strongly encouraged to provide any relevant documentation, recommendations, or limitations relating to your condition.
We reserve the right to adapt, postpone, or refuse sessions in the absence of such information where this is considered necessary for your safety.
9. Hands-On Guidance
Pilates is a precise practice, and we may use hands-on guidance to support alignment and help you move more effectively.
By attending sessions, you consent to this type of contact.
If you prefer not to be touched, simply let us know — this can be adjusted at any time.
10. Safety & Studio Etiquette
To keep the space calm, clean, and safe for everyone:
Grip socks are recommended
Outdoor shoes are not allowed in the studio area
Equipment should be used with care
Phones are best kept aside during sessions
We may refuse or stop a session if behavior or conditions make it unsafe to continue.
11. Responsibility & Participation
By attending sessions, you acknowledge that:
You are participating voluntarily
You take responsibility for your own body and limits
You will follow instructions and communicate openly
You understand that physical exercise carries inherent risks
You are always free to pause or stop any movement or session.
12. Liability
To the fullest extent permitted under the laws of Luxembourg:
By booking or attending a session, you acknowledge and agree that:
– You participate voluntarily and at your own risk
– You remain solely responsible for obtaining appropriate medical advice prior to participation
– You assume full responsibility for any risks, injuries, or damages arising from participation
You agree to waive and release FORMA – Private Pilates Practice, its instructors, and any associated representatives from liability for injuries, damages, or losses arising from participation in sessions, except in cases of gross negligence or willful misconduct.
This includes, but is not limited to:
– Muscle soreness or strain
– Minor injuries
– Fatigue or physical discomfort
Nothing in these Terms excludes or limits liability where such exclusion is not permitted under Luxembourg law.
13. Refunds & Transfers
All sessions and packages are non-refundable
Sessions are non-transferable unless agreed otherwise
Unused sessions expire after the validity period unless extended in advance
14. Privacy
We only collect the information needed to run your sessions smoothly — such as contact details and relevant health notes.
Your data is handled in accordance with the General Data Protection Regulation (GDPR).
Health-related information is treated as sensitive data and is used solely for the purpose of delivering safe and appropriate sessions.
15. Final Notes
These Terms are governed by the laws of Luxembourg.
These Terms constitute the entire agreement between you and FORMA regarding the services provided.
We may update these Terms from time to time. The latest version will always be available on our website.
